Primary Health Network — Northern region [placeholder for actual PHN name pending Chair verification]. Regional PHN with a large palliative-care commissioning envelope and a documented rural/remote population burden.
Who this is for. Laurence owns the relationship (his practitioner voice anchors the ask). Corrina couriers. Carla signs the MOU compliance envelope.
Parents. /health-referrer-mou-pack (the general MOU pack from which this brief specialises) · /launch-90-days (parent sprint pack) · /launch-week-emails (Email #4 opens this pathway).
Compliance envelope. Every artefact preserves: ABN, ACN, ACL 387398, mediation disclosure, complaints/privacy links, dignity floor. No beneficiaries are named or identified. Y1 audited numbers verbatim when cited.
Regional/rural populations in the PHN North catchment are documented to carry disproportionate financial hardship at end-of-life. The Y1 audited pilot cohort included cases from equivalent regional populations, demonstrating the pathway works outside metro contexts.
| Y1 audited outcome | Relevance to PHN North |
|---|---|
| 31 cases | Audited pilot cohort. Every case at intake had a terminal diagnosis in the target population. |
| $1.82M debt resolved | Average $58,700 per case — real scale of the problem the pathway addresses. |
| 23-day average resolution | Inside the medically-meaningful window for most terminal-diagnosis prognoses. |
| 52% average K10 drop | Validated psychological distress instrument — quality-of-life outcome. |
| $1,840 average cost per case | Efficient use of funder resources; sustainable at national scale. |
| 87c direct-service ratio | Every dollar donated funds 87 cents of licensed mediation practice. |
Individual outcomes vary. These describe past pilot results only and are not a guarantee of individual future outcomes.
[Laurence practitioner-voice framing pending · his 30-year mediation-practice framing for PHN North's audience. Substitution machinery from Sprint 55 will land his register.]
PHNs operate on commissioning cycles. Objection risk: 'not this year' — the commissioning cycle for palliative-care services may already be locked for 2026-27. Frame the ask as recognition first, commissioning conversation second. The recognition doesn't need a budget line.

Subject: LWD referral pathway — MOU pack for PHN North.
[Recipient name],
Following [the introduction via {referrer if warm} / the launch announcement of 1 October], I write to formally deliver the MOU pack for the LWD referral pathway, for PHN North's consideration.
[Laurence practitioner-voice paragraph pending]
Life Without Debt went live as a registered Australian charity on 1 October 2026. LWD funds licensed debt mediation for Australians with a terminal diagnosis. The mediation itself is performed by Credit Mediation Services under Australian Credit Licence No. 387398, where I have practised for the past thirty years.
The ask is not endorsement. The ask is recognition of a referral pathway that PHN North may refer into where a patient's care plan identifies financial hardship as a domain of concern. The pathway is set out in the enclosed MOU pack.
The audited Y1 pilot outcomes — 31 cases, $1.82M debt resolved, 23-day average resolution, 52% average K10 psychological distress reduction, $1,840 average cost per case, 87 cents of every dollar to direct-service mediation — are described in the pack. Individual outcomes vary; these are past pilot results only.
I welcome an initial call in the fortnight following delivery. My direct is [phone].
Kind regards,
Laurence Mackay

What to escalate immediately. If PHN North raises a reputational concern about the ACL 387398 pathway, the practitioner-model claim, or the Y1 audit rigor, that is a Carla-and-Chair-immediately concern — do not respond same-day. Route to G4 for a considered response within 5 business days.
